HICAPS settles funds into your bank account in batches that rarely line up one-to-one with a day's appointments, and rejected or partial claims are common, an incorrect member number, a lapsed policy, an annual limit reached mid-treatment. If reconciliation only happens at month-end, the patient has usually already been marked "paid" in your practice management software weeks earlier, even though the fund payment never actually landed.
